- Abstract
Objective: To retrospectively analyze the postoperative pathological features and the changes of Gleason score in patients with positive single needle. Methods: From September 2018 to June 2020, 86 patients with prostate cancer who underwent biopsy in the Urology Department of our hospital and were pathologically diagnosed as single-needle positive were collected for research. Grouping by cTNM, including: age group (age ≥ 70 years old, age < 70 years old), preoperative PSA group (PSA < 10 ng/mL, ≥ 10 ng/mL), preoperative Gleason score group (6 points, 3+4=7 points, 4+3=7 points, ≥8 points), clinical stage group (cT1 stage group, cT2 stage group). All patients underwent laparoscopic radical prostatectomy and single-needle biopsy. The baseline data of the included patients were observed, and the differences and clinical significance of postoperative pathological features in different groups of patients were compared and analyzed. Results: There was no difference in the pathological characteristics of patients of different ages. The proportion of patients with Gleason score ≥ 7 points and Gleason score upgrade in the PSA ≥ 10 ng/mL group was significantly higher than that in the PSA < 10 ng/mL group (P < 0.05); low (P<0.05); the proportion of patients with Gleason score ≥ 7 points in the cT2 stage group was significantly higher than that in the cT1 stage group (P<0.05), but there was no statistical significance in the positive surgical margin and Gleason score upgrade between the two groups (P> 0.05); there was no significant difference in postoperative pT0, positive surgical margin, and Gleason score upgrade between patients with different Gleason scores (P>0.05); there were statistically significant differences in postoperative pathological stages between patients with different Gleason scores, and the Gleason score was 4+3= The pathological stage of patients with 7 points >2 was significantly higher than that of 6 points, 3+4=7 points and ≥8 points (P<0.05); Logistic regression analysis showed that age >70 years old, PSA≥10 ng/mL, cT2 stage and Gleason score ≥ 7 points was the main risk factor affecting the pathological features after operation (P<0.05). Conclusion: Patients with positive single needle needles in the prostate have increased Gleason scores after surgery, and some patients have positive surgical margins and pathological stage ≥ T2. Therefore, it is not possible to use positive single needle needles as a low-risk detection index for prostate cancer patients in clinical practice. An individualized treatment plan will be carried out based on other indicators of the patient.
