- Abstract
The drug prescription is a document that must contain all the information to identify the patient and deliver the correct treatment. The objective of this study was to determine the degree of compliance of hospital prescriptions to the technical and legal standards established by the World Health Organization and Brazilian legislation. This is a cross-sectional and descriptive study of public inpatient prescriptions issued in a small hospital, in August and September 2010. In 139 prescriptions, there were 703 drugs, resulting in an average of 5.06 drugs/prescription. The name of the patient was on 94.25% of the prescriptions, 55.40% identified the ward and bed and the date was present on all of them, whereas the physician's signature was found on only 55.40% and none showed his/her CRM (Regional Medicine Council) registration number. The dosage, administration route, dosage form and frequency/times of administration were present on, respectively, 24.46%, 66.19%, 1.44% and 88.49% of the prescriptions. The Brazilian Common Denomination (government approved generic name) was used for only 41.11% of the drugs. Antibiotics accounted for 10.53% of the drugs and 47.08% were injectable. Drugs for the nervous system were the most frequently prescribed class. Finally, it was proposed that strategies should be adopted to stimulate the correct filling of the prescription, in order to reduce the number of health problems arising from medication errors. Studies identifying information missing from prescriptions may help to change this scenario, while the dissemination and discussion of the data collected in different hospitals could lead to the implementation of plans to reverse the situation.
