- Abstract
Introdução: O sialadenoma papilífero (SP) é uma neoplasia salivar benigna rara, comamaioria dos casos se originando nas glândulas salivares menores. Sua etiologia aindanãoestá esclarecida. Compreende entre 0,4% a 1,2% de todos os tumores de glândulassalivares menores. A localização mais frequente do SP é no palato duro (58%), frequentemente afetando mulheres adultas/idosas. Dois subtipos distintos de SPsãoreconhecidos: o clássico e oncocítico. Microscopicamente, possui características deproliferação papilar exofítica e proliferação ductal subjacente. A maioria dos SPs sãotratados por cirurgia excisional conservadora. O SP, ocasionalmente, pode simular umaproliferação maligna e requer cuidadosa análise microscópica, complementada por estudoimunoistoquímico. O diagnóstico clínico diferencial deve incluir o papiloma escamoso, carcinoma verrucoso, cistoadenoma papilar e disqueratoma verrucoso. Opapilomaescamoso é o diagnóstico clínico diferencial mais comum. Conduta Clínica: Pacientefeminino, de 80 anos de idade, foi encaminhada apresentando lesão nodular, avermelhada, de consistência mole e formato arredondado, superfície rugosa ebasepediculada, localizada no lado direito do palato duro. O diagnóstico clínico favoreceugranuloma piogênico. Foi realizada biópsia excisional. Resultados: o examehistopatológico, suportado pela análise imunoistoquímica (positividade paraCKAE1/CKAE3, CK7, EMA, p40, p63 e raras células para Ki-67; negatividade para p53), foicompatível com SP, clássico, com atipias celulares focais. Conclusão: O conhecimento das características clinicopatológicas do SP contribui para um diagnóstico correto, comimpacto terapêutico e prognóstico.
