- Abstract
Drooling or sialorrhea refers to an unintentional loss of saliva and other oral contents from the mouth. Basically saliva control management in children was done by five approaches. (1) Conservative management-Chronic sialorrhea is seen in children with abnormal oral sensation and motor control and more infrequently when there is excessive production of saliva. Salivary functions from the paired glands is under autonomic parasympathetic control. Management of problem relies on multidisciplinary team with focus on assessment with appropriate conservative interventions, oral motor training. (2) Oral appliances - Fixed and removable orthodontic appliances can be used for saliva control in children. (3) Medication including oral-systemic review of anticholinergic drugs-Benzotropine, Glycopyrrolate and Benhexol Hydrochloride are effective in treatment of drooling. But these drugs have side effects. Hyoscine-The effect of oral anticholinergic drugs have been limited in treatment of drooling, Glycopyrrolate Antimuscarinic drugs-such as benhexal Antireflux medication. Modafinil Alternate medication Papaya and Grape seed extract. 4) Botulinum Toxin Injections-Doctors inject botulinum toxins type A (Botox) into the children's salivary gland to treat excessive drooling. Botox reduces the activity of the salivary glands. It leads to less saliva production. Surgical management Salivary Duct Ligation Rerouting the parotid or submandibular ducts. Excision of the submandibular glands. Transection of the nervus innervating the parotid gland.
