TOLERANCIA A ALIMENTOS ESPECÍFICOS EN DIETAS PROGRESIVAS TRAS BYPASS GÁSTRICO.

- Abstract
Introducción: El bypass gástrico (cirugía derivativa o parcialmente malabsortiva) es uno de los tratamientos coadyuvantes en la obesidad mórbida. Alrededor del 30% de los pacientes sometidos a cirugía para perder peso desarrollan complicaciones relacionadas con la ingesta de determinados alimentos como: náuseas, vómitos e intolerancia a ciertos alimentos y deficiencias nutricionales, como anemia y osteoporosis entre otras. Objetivo: -- Valorar la tolerancia a alimentos concretos y sus cocciones tras cirugía bariátrica, para disminuir el riesgo de padecer alteraciones nutricionales y mejorar la calidad de vida de estos pacientes. -- Diseñar un programa dietético óptimo para estos pacientes. Métodos: Estudio descriptivo y retrospectivo de 50 pacientes con obesidad mórbida de nuestra serie, sometidos a bypass gástrico para valorar las intolerancias a alimentos específicos y sus cocciones. Se utilizó entrevista dietética individualizada y registro alimentario, a los 3, 6, 12 y 18 meses hasta los 2 años postintervención. Con todos los datos estudiados se realizó un análisis descriptivo utilizando el paquete estadístico SPSS versión 8.0. Resultados: De los 50 pacientes estudiados un 32% fueron varones y 68% mujeres, con una edad media de 42,4 ± 9,3 años (26-59), y un IMC medio previo a la cirugía de 45,19 ± 5,1 kg/m² (35, 7-56,9). Conclusiones: -- Existe un elevado porcentaje de pacientes que no tolera ciertos alimentos tras bypass gástrico. -- Las cocciones secas (plancha, brasa) son las que peor se toleran. -- Es necesario realizar entrevistas dietéticas individualizadas después de una cirugía bariátrica para detectar posibles intolerancias y poder evitar o reducir el riesgo de padecer deficiencias nutricionales. -- El resultado de este registro nos ayuda a diseñar un programa de progresión de dietas tras este tipo de cirugía
