Occupational sensitization to aminopenicillins with oral tolerance to penicillin V.

- Abstract
A 45-year-old nurse gave a 20-year history of recurrent vesiculation and erythema of the sides of the fingers of her right hand, which spread to the back of the hand, with scaly resolution, within a 1-week period. She attributed her condition to handling either ampicillin or amoxycillin vials for parenteral use, but not to handling benzylpenicillin - or any other drugs. Standard patch tests were negative. The patient showed an exclusively delayed sensitization to aminopeniciflins as suggested by her history and as demonstrated by both i.d. and patch tests. In addition, immunologic specificity was demonstrated by the patient tolerating oral phenoxymethytpenicillin, a molecule that differs from ampicillin only in the lack of an amino group on the benzene ring.
