Transient side-changing hemispheric dysfunction: an unusual presentation of bilateral thalamic infarction mimicking large vessel occlusion.

- Abstract
This article describes a case study of a 70-year-old woman who presented with acute onset blurred vision and subsequently developed symptoms of left hemispheric dysfunction, including aphasia, right gaze palsy, and right hemiplegia. The initial diagnostic work-up ruled out intracranial hemorrhage and large vessel occlusion. However, the patient's symptoms spontaneously improved and then worsened, leading to the discovery of an acute bilateral thalamic infarction. The authors suggest that the transient and side-changing nature of the symptoms may be due to a temporary disruption of thalamo-cortical networks rather than pure anatomical damage. Thalamic infarction should be considered as a differential diagnosis in cases of acute onset hemispheric dysfunction.
