A INTEGRALIDADE DA ASSISTÊNCIA A PACIENTES COM HIV EM AMBULATÓRIO ESPECIALIZADO DE ALAGOAS.

- Abstract
Objective: In the complex relationship between health and disease, is undeniable the value of the integral care. Patients with the human immunodeficiency virus (HIV) may have renal disorders influencing the prognosis. The objective is to analyze the search for evidence of renal impairment, by professionals following patients with HIV, as an aspect of integral care. Patients and Methods: retrospective analysis of medical records and prospective collection of samples, including 99 infected patients from an outpatient service for HIV/Acquired immune deficiency syndrome, in a hospital of infectious diseases. Thirty-one patients were interviewed and had collection of urine samples for evaluation of microalbuminuria. Glomerular filtration rate was estimated by the simplified formula of modified diet in renal disease (MDRD). Results: of the 99 patients, 58 (58.58%) were men and 41 (41.41%) women; 84 (84.84%) mulattos, 10 (10.10%) black and five (5.05%) white; with a mean age of 41.02 ± 11.05 years. As to the treatment, lamivudine (3TC) was the most used drug (84.21%); and 26 patients (21.37%) used the 3TC association, zidovudine and efavirenz. It was clearly realized that the search for the diagnosis of renal involvement was not significant. Among 95 patients, only 17 (17.89%) had result of simple urine test in the chart. Of these, two (11.76%) had change in color, eleven (64.70%) had change in the appearance, and three (17.64%) had pH less than 5.5; one had pH higher than 7.0 (5.88%) and three (17.64%) had proteinuria of 1+. Seven (7.36%) had hypertension, five (5.2%) reported urinary complaints and three (3.15%) showed edema. The average of microalbuminuria in the isolated sample was 2.12 ± 3.32 mg/dL and 12 patients (38.71%) showed values above 1.7 mg/dL. Conclusion: in the studied population there was no application of integral care in practice, even dealing with people at risk of nephropathy.
