- Abstract
The tubulo-interstitial nephritis (T-IM) is a group of diseases in which after the activation of a specific damaging factor, early morphological changes are located only within the renal tubules and in the interstitial tissue. The most important cause of inflammation, not related to infection, is the hypersensitivity reaction to drugs (about 70% of cases of (T-IM) confirmed by kidney biopsy). The poisoning of fungi containing nephrotoxic compounds (orelanin found in some Cortinarius species, amatoxins in Amanita phalloides, etc.) is not common, but due to the dramatic course and lack of a specific antidote need attention. The paper presents the case of a patient with acute tubulointerstitial nephritis confirmed by biopsy, caused by poisoning of fungi from the family Boletus, which occurred during his stay in the US A. Implementation of glucocorticosteroid treatment may reduce the risk of developing renal failure in (T-IM) with toxic aetiology, which is also the case in the case of the patient. The applied treatment resulted in a very large improvement in kidney function, and the lack of complete remission was most likely a consequence of the late start of GKS therapy.
