- Abstract
A Guidant PRIZM VVIR ICD modified for biventricular ICD function with a Y-connector with the right ventricular (RV) lead as the cathode and the left ventricular (LV) lead as the anode was implanted in a patient with permanent atrial fibrillation (and ablation of the AV junction), refractory congestive heart failure, dilated cardiomyopathy, left bundle branch block and ventricular tachycardia. The recording obtained during an episode of severe dizziness shows a double jeopardy phenomenon. Chest X-ray film showed slight proximal displacement of the LV lead in an anterolateral coronary vein. The experiment illustrates that far-field atrial sensing by a biventricular ICD with common ventricular sensing is more serious in patients who have undergone AV junctional ablation.
