- Abstract
This is a 12-month randomized controlled trial, evaluating the use of telehealth (remote patient monitoring and video visits) versus standard transitions of care with the outcomes of over-utilization, access to care, medication management, patient adherence and patient engagement. Such solutions combining predictive analytics, remote patient engagement, and telehealth facilitate the delivery of value-based care, improve patient health outcomes, and decrease healthcare costs. Through the mobile application, education is delivered to the patient, the patient is prompted to report symptoms and vitals, and patients are able to send a secure message to their provider. B Conclusions: b While the service is only currently reimbursable for Medicare patients, this standardized virtual check-in process is available to all established patients as a method for providers to follow-up with their patients outside of the clinic. In order to support our belief that our cardiology patients would be willing to be seen via telehealth technology, we determined that we would survey the patients prior to the telehealth visit and then after the visit to see if there was any change in attitude towards using telehealth services by our cardiology patients.
