INFECÇÃO FÚNGICA POR CRYPTOCOCCUS SP. EM FELINO DOMÉSTICO - RELATO DE CASO.

- Abstract
A Criptococose é uma das doenças fúngicas que mais acometem gatos, causada principalmente pelas espécies Cryptococcus (C.) neoformans e C. gattii. O hospedeiro é infectado após a inalação de esporos do ambiente e posterior colonização do agente, principalmente em cavidade nasal, pele, pulmões, gânglios linfáticos e sistema nervoso central. Foi atendido em Hospital Veterinário, um felino, fêmea, sem raça definida, pesando 2,6 Kg, 3 anos de idade. Ao exame clínico, observou-se lesão ulcerada em região dorsal da cabeça, medindo 8 cm, alopécica, irregular, sem áreas de necrose visíveis, sendo trauma contundente a suspeita diagnóstica inicial. Na cabeça apresentava edema de tecidos moles e enfisema subcutâneo em calota craniana, radiograficamente não demonstrando comprometimento ósseo, apresentando secreção em seios nasais bilateralmente, sem secreção macroscópica. Foi solicitada citologia da lesão, inicialmente corada com Panóptico, a qual demonstrou estruturas leveduriformes de formato arredondado, envolto por cápsula de mucopolissacarídeo, sugestivas de fungos. Para confirmação diagnóstica, foi realizada cultura micológica da lesão, em meio ágar Sabouraud com cloranfenicol e Mycobiotic Agar, incubado por 7 dias a 37º graus. Após este período, em ágar Sabouraud, houve o crescimento de uma colônia cremosa e mucoide, de coloração branca, compatível com Cryptococcus sp. Utilizou-se o corante Nanquim e Azul de Lactofenol para visualizar microscopicamente as estruturas encapsuladas, compatíveis com o agente. A importância desta determinação é diferenciar lesões causadas por esporotricose, importante zoonose, e a criptococose, lesão fúngica mais frequente em felinos que não acomete humanos, já que ambas lesões macroscópicas e suas apresentações citológicas são muito semelhantes. Após quatro meses de tratamento, paciente apresentou redução significativa da lesão, que após submetida a coleta de nova amostra para cultivo fúngico, não apresentou crescimento. A doença nasossinusal crônica é o achado clínico mais comum na criptococose, podendo ser vista isolada ou em conjunto com disseminação local para tecido subcutâneo, ossos e linfonodos, além de induzir um edema nasofacial e uma ulceração não cicatrizante com secreção nasal.
