Avanços nas modificações antimicrobianas de superfície de implantes dentários: desafios e perspectivas promissoras contra biofilmes.

- Abstract
Introdução: Infecções peri-implantares são uma das principais causas de falha de implantes dentários e são iniciadas por biofilmes disbióticos polimicrobianos na superfície do implante. A baixa eficácia dos tratamentos disponíveis aponta para a necessidade de novas estratégias de prevenção, modulação e eliminação de biofilmes dessas superfícies. Objetivo: Discutir criticamente os mecanismos antimicrobianos de superfícies modificadas de implantes dentários de titânio e as técnicas empregadas para alcançá-las. Material e Método: Realizou-se busca nas bases de dados MEDLINE/PubMed e Web of Science para levantamento do referencial teórico. Foram incluídos ensaios in vitro, in vivo, clínicos e revisões sistemáticas de literatura. Resultados: Diversas modificações de superfície foram propostas usando biomoléculas, íons, polímeros, agentes antimicrobianos e alterações de topografia. Os mecanismos antimicrobianos dessas superfícies são de morte por contato, liberação de agentes bactericidas ou bacteriostáticos, efeito antiadesivo ou combinação entre eles. Dentre as principais técnicas de modificação de superfície estão o layerby-layer, plasma eletrolítico de oxidação, anodização eletroquímica e deposição eletroforética. As modificações antimicrobianas de superfícies demonstraram resultados microbiológicos eficazes; no entanto, não há consenso sobre a melhor estratégia e faltam informações detalhadas sobre a segurança, com relação à citotoxicidade e a longevidade do efeito anti-infeccioso, distanciando essas superfícies da aplicação clínica. Conclusão: Apesar de eficazes, superfícies antimicrobianas apresentam desafios recorrentes, como eficácia de curto prazo, liberação exacerbada de agentes antimicrobianos, citotoxicidade e ausência de reusabilidade. Em contrapartida, superfícies responsivas a estímulos parecem ser uma estratégia promissora para efeito controlado e preciso, devendo ser investigadas em modelos que melhor mimetizem condições clínicas.
