- Abstract
INTRODUCTION: Transanal endoscopic microsurgery (TEM) allows for complete local excision of benign rectal tumors and T1 carcinomas, as well as for palliative treatment for advanced rectal carcinomas. Presently, local excision of selected rectal tumors is accepted worldwide as the standard medical care. Compared with conventional surgery it has comparable oncological and better postoperative results, morbidity, mortality and quality of life. In this paper we present the three-year experience of our clinic in the treatment of rectal cancers by transanal endoscopic microsurgery. METHODS: For the period from October 2010 to October 2013 year at our clinic 52 patients with rectal cancers underwent TEM. The following surgeries were performed: complete excision for 26 patients with T1 rectal carcinoma, palliative for 20 patients and haemostasis for 6 patients depending on the stage of advancement of the disease and complications occurred. The cases are discussed in prospective plan. RESULTS: All operations were completed without going to conversion. In three cases in the early postoperative period (24-48 hours) laparoscopic surgery was performed. The duration of operative time ranged between 90 and 100min. The average blood loss was 50-60 ml. Postoperative hospital stay was 1-3 days. Postoperative complications were observed in three patients (6%). In the follow up of our patients to date we have not observed a recurrence after a radical TEM and mortality is 0%. CONCLUSION: The application of TEM in rectal cancer treatment is characterized by a shorter hospital stay, an insignifi cant blood loss, and low rates of recurrence and mortality. One of the signifi cant advantages of TEM is that the patients maintain their normal quality of life. The application of TEM as a radical method of treatment requires an exact preoperative assessment of the T-and N-stage malignancies.
