DIABETES MELLITUS GESTACIONAL: DETECÇÃO E MÉTODO DE MONITORAMENTO EM TEMPOS DE PANDEMIA POR COVID-19.

- Abstract
INTRODUÇÃO A pandemia por COVID-19 exigiu rápida transformação e adaptação dos serviços de saúde em todo o mundo para garantir atendimento médico adequado para muitos grupos de pacientes, e isso inclui mulheres com diabetes mellitus gestacional (HICK & BIDDINGER, 2020; WILK et al., 2021). No início de 2020, o pré-natal de gestantes com diabetes foi rapidamente redesenhado para criar modelos flexíveis de atendimento, detecção e monitoramento (PEAHL et al., 2020). OBJETIVO O presente trabalho visa identificar os métodos de detecção e monitoramento do Diabetes Mellitus Gestacional durante a pandemia por COVID-19. METODOLOGIA Trata-se de estudo descritivo, de revisão integrativa da literatura, que contou com a filtragem de artigos em periódicos cadastrados nas bases de dados das plataformas PubMed e Scielo. Os descritores foram utilizados de forma combinada e e/ou isolada, sendo estes: Diabetes mellitus gestacional, COVID-19 e Cuidado Pré-Natal. Os critérios de inclusão abrangeram artigos originais na íntegra e gratuitos, publicados em 2020 e 2021, na língua inglesa e/ou portuguesa. Foram excluídos estudos que não se enquadraram nos parâmetros, artigos de revisão de literatura e estudos de caso. Para nortear a pesquisa, foi elaborado o seguinte questionamento: Quais métodos de detecção e monitoramento do Diabetes Mellitus Gestacional foram recomendados na pandemia por COVID-19? A leitura e análise de cada artigo foi criteriosa sendo, posteriormente, elaborada uma planilha com objetivo de sintetizar e organizar os dados no software Microsoft Excel. RESULTADOS Foram localizados 55 estudos nas bases de dados analisadas. Deste total, 38 artigos foram excluídos por não se enquadrarem nos critérios de inclusão estabelecidos e 17 artigos foram analisados. Quanto ao delineamento, 53% dos estudos eram quantitativo descritivo, 47%, qualitativos e todos foram escritos na língua inglesa. Houve predominância de 65% para artigos publicados em 2021 e 35% para 2020. Quanto ao método de detecção da Diabetes Mellitus Gestacional, os autores citaram exames laboratoriais que poderiam ser utilizados em alternativa à aplicação do Teste Oral de Tolerância à Glicose, como: teste de glicemia em jejum, teste aleatório de glicose plasmática, hemoglobina glicada e exame de urina. Para 33% dos autores, o Teste Oral de Tolerância à Glicose não deveria ser excluído, devendo ser mantido mesmo em período de pandemia por COVID-19. Enquanto que para 7% dos autores, a aplicação do Teste de Tolerância deveria ser excluída no período, por tratar-se de exame de longo período de exposição em ambiente externo ao domicílio e risco de atendimento médico durante exame. No que diz respeito ao método de monitoramento, todos os autores citaram o uso de telemedicina como forma de auxiliar o acompanhamento do paciente, além de práticas de educação nutricional com o nutricionista para o autogerenciamento da diabetes. CONCLUSÃO Mediante a análise efetuada, os estudos apontam que houve uma leve alteração nos exames de detecção de Diabetes Mellitus Gestacional durante a pandemia do novo coronavírus. Quanto ao monitoramento, a telemedicina demonstrou ser o principal instrumento utilizado no acompanhamento das pacientes, e além disso, notou-se a notória importância do nutricionista nas ações de autogerenciamento da diabetes.
