- Abstract
Despite the remarkable advances in the medical management of patients with pulmonary hypertension, in the majority of patients the disease continues to progress and will require lifesaving lung transplantation. Lung transplantation remains the last therapeutic option in patients affected by pulmonary hypertension whose disease progresses despite maximal medical therapy. Lung transplantation for IPAH has traditionally been viewed as a procedure with a higher up-front surgical risk; considering complexity of the procedure and the shortness of lung donors it is important to optimize the timing for listing and the management of the patients on the waiting list. A close communication between the primary team and the lung transplant team is crucial to optimize medical therapy and to monitorize every sign of decompensation. The waiting list mortality for patients with IPH is mostly due to right heart decompensation; an option to support these patients waiting for lung transplantation is the use of Extracorporeal Lung Support (ECLS). Despite the high complexity of lung transplantation in patients with IPH, the survival figures are acceptable and lung transplantation remains the most balanced options for this subset of patients.
