- Abstract
Wstęp: Chorzy z alergią na pokarmy stanowią heterogenną grupę. Zróżnicowane są przyczyny objawów, patogeneza i obraz kliniczny. Podstawą podejrzenia alergii pokarmowej (AP) jest wywiad, tylko część objawów można ocenić obiektywnie w oparciu o badanie przedmiotowe, testy in vitro i in vivo weryfikują IgE zależny mechanizm nadwrażliwości, ale nie ma między nimi pełnej korelacji; niedostępne są metody laboratoryjnej weryfikacji alergii niezależnej od IgE. Brak precyzyjnych metod diagnostycznych uniemożliwia personalizację zaleceń terapeutycznych. Tych problemów nie rozwiązują także stosowane od kilku lat nowe systemy diagnostyczne oparte na diagnostyce molekularnej. Wciąż zauważalna jest potrzeba wprowadzenia nowych markerów diagnostycznych oraz poszukiwania skutecznych celów terapeutycznych. Jednym z głównych czynników ryzyka rozwoju AP jest atopowe zapalenie skóry (AZS). AZS jest zazwyczaj uznawane za paradygmat choroby alergicznej, w której pośredniczą limfocyty Th2 i Th1. Natomiast ostatnie odkrycia naukowe sugerują, również udział komórek T pomocniczych typu 17 (Th17) w patogenezie alergii i AZS. Cel: Celem badania było oznaczenie stężenia cytokin prozapalnych zaangażowanych w szlak odpowiedzi immunologicznej komórek T pomocniczych typu 17 w surowicy atopowych dzieci z IgE-zależną oraz IgE-niezależną AP. Cytokiny będąc ważnymi mediatorami odporności stanowią obiecujący cel terapeutyczny oraz mogą być rozważane w kontekście potencjalnych czynników prognostycznych. Materiały i Metody: Do badania włączono 76 dzieci (0-5 lat) z przewlekłymi objawami AZS. Dodatkowo u części pacjentów wystąpiły ostre objawy po ekspozycji na pokarmy pod postacią pokrzywki, obrzęku naczynioruchowego i/lub anafilaksji. Na podstawie historii choroby, objawów oraz wyników molekularnego badania ALEX2 pacjenci zostali przydzieleni do odpowiedniej grupy: IgE--zależna AP; IgE-niezależna AP; grupa kontrolna (brak AP). Surowice pacjentów zostały poddane badaniu Bio-Plex Pro Human Cytokine Assay. Nowatorska technologia Luminex/XMAP, umożliwiła jednoczesne oznaczenie stężenia 15 cytokin w jednym eksperymencie. Otrzymane dane poddano analizie statystycznej wykorzystując test Kruskala Wallisa, ANOVA oraz krzywe ROC (p<0.05). Wyniki: Stężenie cytokin zapalnych w surowicy pacjentów z IgE-zależną AP było zwiększone w porównaniu z pozostałymi grupami. Różnica była istotna statystycznie dla IL-1b. W grupie pacjentów z IgE-niezależną AP ekspresja większości cytokin była obniżona porównując z grupą kontrolną i uczestnikami z alergią zależną od IgE. Zgodnie z krzywymi ROC, interleukina 1 beta może być rozważana jako potencjalny czynnik predykcyjny IgE-zależnej AP u małych dzieci z przewlekłymi objawami AZS (AUC~0.7;p<0.05). Podsumowanie: Nasze wyniki sugerują, że występowanie IgE-zależnej alergii pokarmowej znacząco wpływa na profil cytokin zaangażowanych w szlak odpowiedzi immunologicznej komórek Th17 u pacjentów z przewlekłymi objawami AZS, zwiększając ich ekspresję. Cytokiny te mogą brać udział w patogenezie IgE-zależnej AP. Podwyższone stężenie IL-1 beta w surowicy może być predyktorem alergii pokarmowej w przebiegu AZS. Interleukinę 1beta należy, również uznać za potencjalny cel terapeutyczny. IL-1RA może blokować wiązanie IL-1 beta z receptorem, hamując kaskadę immunologiczną, związaną z odpowiedzią limfocytów Th17. Wymagane są dalsze, zawansowane badania na poziomie proteomicznym, aby potwierdzić nasze założenia. Z pewnością otrzymane dane rzucają nowe światło na rolę i zaangażowanie cytokin prozapalnych w reakcję alergiczna i odpowiedź immunologiczną organizmu w przebiegu AZS.
