Confined space entry -- asphyxiation of contract worker.
Abstract
The article focuses on the asphyxiation of employees upon entering a confined space. It presents two cases of asphyxiation which involve employees wherein one occurred in 1991 and involves a welder who entered a storage area without a breathing apparatus. Furthermore, the other incident happened in 2005 when a worker and a would-be rescuer were asphyxiated during the installation of a temporary supply of nitrogen on a hydrocracker unit reactor.
