- Abstract
O uroperitônio é uma afecção que pode acometer neonatos na espécie equina, sendo mais comum em animais sépticos ou mal-ajustados. Caracterizado pelo extravasamento de urina para a cavidade abdominal, tem origem primária traumática pela compressão da vesícula urinária ou úraco durante a passagem do neonato pelo canal do parto, ou ainda secundária a onfalites. O seguinte relato refere-se a um neonato de 9 dias, macho, da raça Friesian, de 40 kg, que chegou apresentando quadro de letargia e desidratação. Durante exame físico, contatou-se elevação do tempo de preenchimento capilar (3 segundos), distensão abdominal (ascite), taquicardia (100 bpm), frequência respiratória de 20 mpm, dor abdominal severa, disúria e dificuldade para se manter em estação. Além disso, apresentava sinais de encefalopatia, como dificuldade de levantar, incoordenação e ausência de reflexo de sucção. Os exames laboratoriais indicaram uremia (143,9 mg/dL) e leucocitose severa (30.000/dL). O exame ultrassonográfico apontou grande volume de líquido livre na cavidade abdominal, não sendo possível a visualização da vesícula urinária. Realizou-se sondagem uretral, com recuperação de 20ml de urina, seguido de drenagem por paracentese, sendo obtido 4 litros de líquido com coloração amarelada característico de urina. A descompressão abdominal, associada à fluidoterapia com solução fisiológica e ao controle dos eletrólitos (sódio e potássio), permitiu que houvesse a diminuição da concentração de ureia abdominal, apresentando melhora no quadro clínico, reduzindo a letargia e recuperando o reflexo de sucção. O animal foi então submetido à laparotomia exploratória, na qual diagnosticou-se ruptura de vesícula urinária no local de inserção do úraco. Realizou-se a correção da ruptura de vesícula, utilizando o padrão de sutura Parker-Kerr, com fio ácido poliglicólico 2-0, e o úraco foi removido junto ao umbigo. Prosseguiu-se a lavagem da cavidade abdominal com ringer lactato para remover resquícios de urina. Realizou-se sutura simples contínua com ácido poliglicólico 2 para fechamento da musculatura, seguido de padrão contínuo zigue-zague com ácido poliglicólico 2-0 em subcutâneo e sutura simples contínua com fio Nylon 1 para síntese da pele. O animal apresentou intercorrências durante a anestesia, sofrendo uma parada cardiorrespiratória, sendo realizado protocolo de ressuscitação cardiopulmonar com êxito. Mantevese a sonda uretral nas 12 primeiras horas para evitar distensão vesical e foi estabelecido o tratamento pós operatório utilizando ceftiofur (7,5 mg/kg, BID, 10 dias), meloxican (0,6 mg/kg, BID, 5 dias), omeprazol (4 mg/kg, SID, 10 dias), 1 litrL de plasma hiperimune e hidratação com solução fisiológica 0,9% (100 ml/kg/d). Após 3 dias, o nível de ureia sanguínea reduziu para 14 mg/dL, associada à melhora clínica e desenvolvimento do neonato. O potro recebeu alta 20 dias após a cirurgia. Conclui-se que o diagnóstico precoce permite que seja realizada a estabilização adequada do paciente, sendo possível seu encaminhamento para a cirurgia de emergência e, com todo aporte pré, trans e pósoperatório, obter êxito no tratamento.
