- Abstract
Giriş: COVID-19 pandemisi gündemimizi meşgul etmeye devam ederken hastalık prognozunu etkileyen durumlarla ilgili araştırmalar da sürdürülmektedir. Bu durumlardan bir tanesi de mineral dengesindeki bozukluklardır. Bu çalışmamızda kalsiyum (Ca), magnezyum (Mg) ve fosfor (P) düzeylerinin COVID-19 seyrindeki değişimini ve mortaliteye etkisini gözlemlemeyi amaçladık. Gereç ve Yöntem: Recep Tayyip Erdoğan Üniversitesi Eğitim ve Araştırma Hastanesi, Enfeksiyon Hastalıkları Pandemi Kliniği’nde 1 Ocak 2022-31 Mart 2022 arasında takip edilen, yatış tetkiklerinde Ca, Mg, P, albümin olan 18 yaş üstü hastalar çalışmaya dahil edildi. Veriler retrospektif olarak hastane sisteminden tarandı. Hastaların yaşı, cinsiyeti, COVID-19 tanısı öncesi son 2 yılda ve hastane yatış anında bakılan serum P, Mg, düzeltilmiş Ca değerleri hastaların yoğun bakım ünitesi (YBÜ) izlemi varlığı ve mortalite varlığı kaydedildi. Verilerin incelenmesinde SPSS 26.0 istatistik paket programı kullanıldı, istatistiksel anlamlılık düzeyi p<0,05 olarak kabul edildi. Bulgular: Değerlendirilen 149 hasta arasından COVID-PCR pozitif olan 116 hasta çalışmaya dahil edildi. Hastalar çoğunlukla kadındı (n=59, %50,9) ve yaş ortanca değeri 71 yıldı (21.106). Hastaların %66,4’ünün (n=77) yatış Ca, %57,8’inin (n=67) yatış Mg, %65,5’inin (n=76) yatış P değerleri normal aralıktaydı. COVID-19 hastalığı öncesinde bakılan değerlerde ise %66,4’ünün (n=77) Ca, %31,9’unun n=37 Mg, %43,1’inin (n=50) P değerleri normal aralıktaydı. COVID-19 öncesi ve sonrası ortalama değerler karşılaştırıldığında hastalarda COVID-19 döneminde Ca ortalama değerlerinin istatistiksel olarak anlamlı derecede düştüğü, P ve Mg ortalama değerlerinin ise önemli ölçüde değişmediği tespit edildi (sırasıyla; p=0,00, p=0,864, p=0,338). Sonuçlar Tablo 1’de gösterilmiştir. COVID-19 öncesi dönemde Ca, Mg, P değerleri normal aralıkta olup yatış esnasında düşük saptanan hastaların mortalite ve YBÜ takibi ile aynı zamanda hastaların yatış Ca, Mg, P değerleri ile mortalite ve YBÜ takibi karşılaştırıldığında istatistiksel açıdan anlamlı sonuçlara varılamadı. Sonuçlar Şekil 1’de gösterilmiştir. Sonuç: Vitamin-mineral homeostazı vücutta önemli sinyal yolaklarının, metabolik yolakların düzenlenmesinde büyük öneme sahiptir. Ca, P ve Mg düzeylerindeki düşüklüğün veya yüksekliğin COVID-19’un seyrini etkileyebileceği veya ciddi COVID-19 gelişimini öngörmede yol gösterici olabileceğiyle ilgili yapılmış çalışmalar mevcuttur. Biz bu çalışmamızda serum Ca düzeylerinin COVID-19 hastalık dönemine düşebileceğini ancak bunun mortalite ve YBÜ takibi ihtiyacına etkisinin olmadığını bulduk. Hastaların COVID-19 önceki dönemlerde almış olduğu tedaviler, vitamin destekleri gibi bilgilere sistem kayıtlarında olmadığından ulaşılamamış olması çalışmanın kısıtlılıklarındandır. Bu sebeple prospektif yapılacak, daha sık serum düzeylerinin bakılacağı daha kapsamlı çalışmalara ihtiyaç duyulduğu kanaatindeyiz.
