DISFUNCIÓN AUTONÓMICA EN ENCEFALITIS AUTOINMUNE CON ANTICUERPOS CONTRA EL RECEPTOR NMDA QUE INGRESARON A UNIDAD DE CUIDADO INTENSIVO EN MÉXICO.

- Abstract
Objetivo: Describir las disfunciones autonómicas (DA) y complicaciones en pacientes con encefalitis autoinmune con anticuerpos contra receptor NMDA (EAantiNMDAr) ingresados a unidad de cuido intensivo (UCI). Antecedentes: La DA es una manifestación frecuente de la EAantiNMDAr, reportada con prevalencia del 25 a 45%, siendo la taquicardia sinusal (TS), la presentación más frecuente hasta en un 69.9%. Suele ser de mal pronóstico, al relacionarse con infecciones y estatus epiléptico. Métodos: Estudio transversal, descriptivo, incluyó a pacientes con EAantiNMDAr ingresados a terapia intensiva del 2010-2022. Este trabajo es un addendum del protocolo con registro 86/19. Se realizó análisis estadístico con el software IBM SPSS v.26. Resultados: Se incluyó a 21 pacientes, edad media 32.67 años (IC21-58), sexo femenino (69.1%). El 33.3% ingresó por estado epiléptico, el resto por disautonomía. A su ingreso el 71.4% presentó TS, 61.9% presentó hipotensión arterial, 61.9% hiperpnea, 38.1% retención urinaria, descartándose infecciones sistémicas. En urgencias el promedio de la frecuencia cardiaca fue de 93.9 lpm (SD17.6), respiratoria de 19.6 (SD2.2), presión sistólica 118.3 mmHg (SD10.5) y diastólica 74.24 mmHg (SD7.5), a su ingreso a UCI con media de la frecuencia cardiaca de 93.2 lpm (46-164 SD28.8), con frecuencia respiratoria 19.9 (12-28 SD19.9), presión sistólica 120.7 mmHg (70-182, SD22.4) y diastólica 71.4 mmHg (50-92, SD13.3), se compararon las medias de los signos vitales en urgencias y UCI con prueba U de Mann-Whitney, llama la atención los intervalos más amplios de la frecuencia cardiaca, presión arterial y frecuencia respiratoria, en UCI, que a su ingreso a urgencias. Al comparar por etiología de ingreso, los pacientes con estado epiléptico tuvieron mayor presentación de bradicardia sinusal 80% (p=0.011), 85.7% presentó hipopnea central (p=0.005), contradictorio a la baja presentación de desaturación en el 14.3%. Conclusión: En nuestro estudio encontramos una prevalencia similar a lo descrito en la literatura internacional. Dentro de los parámetros indicadores de DA, la TS fue la más frecuente. Hasta 1/3 de nuestros pacientes con TS requirieron ser ingresados la UCI Por lo que puede ser un marcador de mal pronóstico, pudiendo relacionarse infecciones respiratorias y estatus epiléptico. Por lo que se requiere realizar un estudio prospectivo, comparando con grupo control, de aquellos pacientes que no requirieron ingreso a UCI.
