- Abstract
Introduction: To evaluate the predictive factors for bladder recurrence under long-term follow-up of patients who underwent nephroureterectomy for primary upper urinary tract urothelial carcinoma (UUT-UC). Methods: Between July 1994 and June 2005, 230 consecutive patients with primary UUT-UC underwent unilateral nephroureterectomy with bladder cuff resection at our institution, including 50 transperitoneal open, 67 retroperitoneal open, 61 transperitoneoscopic and 52 retroperitoneoscopic surgeries. The medical records of these patients were retrospectively reviewed. The clinical and histopathological data were analyzed to evaluate predictive factors. Results: The mean follow-up time was 83 months. In total, 88 patients (38%) developed subsequent bladder urothelial carcinoma (UC). Under Cox proportionalhazards regression with the significance level of (P = 0.0002), the independent predictive factors for bladder recurrence were history of bladder UC (P = 0.027), operative time (P = 0.032), and presence of ureteral UC (P = 0.016). Impaired renal function, operative method, cancer staging, and cancer grading failed to stand as independent predictive factors. Conclusions: History of bladder UC, operative time, and presence of ureteral UC were independent predictive factors for bladder recurrence after nephroureterectomy for primary UUT-UC.
