For these reasons, and the fact that larger proximal muscles require higher doses, total starting doses can vary significantly but typically range between 30 to 100 units onabotulinum toxin A and 100 to 300 units abobotulinum toxin A per upper limb across 3 to 6 injection sites. Pectoralis major is not routinely injected with the initial treatment unless there is significant adduction-abduction movement of the shoulder with the outstretched arm, but the inclusion of this muscle may be required with subsequent treatments. However, if it has reached the point of causing significant weakness and the patient still has not felt a meaningful improvement in his or her tremor, this constitutes a failure of treatment and ongoing injections are unlikely to help. 10 Samotus O, Lee J, Jog M.
