- Abstract
A piroplasmose equina é uma enfermidade decorrente do parasitismo em células hematopoiéticas ocasionada principalmente pelos protozoários Babesia caballi e/ou Theileria equi. É transmitida por vetores biológicos hematófagos da família Ioxididae (ONYICHE et al, 2019). A Theileria equi é um dos principais hemoparasitas descritos em equinos, possui duas fases no ciclo de infecção, extra e intraeritrocitária, infectando, inicialmente, os linfócitos e, em seguida, as hemácias, diferindo ao ciclo da Babesia caballi, que, por sua vez é predominantemente intraeritrocitário (VIANNA et al., 2020). Este resumo tem por objetivo relatar a associação entre a infecção por piroplasmídeo e a ocorrência de anemia associada à auto aglutinação sanguínea numa potra de 3 meses de idade, sem raça definida, atendida no Hospital Veterinário de Grandes Animais/UFRRJ. O animal apresentava febre, prostração e inapetência, corroborando com os achados em literatura que citam também mioglobinúria, hepato e esplenomegalia (SCOLES & UETI, 2015). Na ocasião, foi realizado hemograma, no qual observou-se anemia microcítica normocrômica com hematócrito em 23% (32 - 48%), VCM 33,9 fL (34-58 fL) e CHCM 33 g/dL (31-37 g/dL), leucopenia por neutropenia de 5200 células/µL (6000 - 12000 células/µL), trombocitopenia com 57000 células/uL, hiperfibrinogenemia de 0.8 g/dL (0,1 - 0,4 g/dL) e presença de raras estruturas compatíveis com Corpúsculos de Howell-Jolly. Na pesquisa de hematozoário, observou-se inclusões em hemácias sugestivas de hemoparasitas da ordem Piroplasmida. Após observação de macroaglutinação no tubo de hemograma, foi feito um teste de hemaglutinação com salina e observou-se intensa microaglutinação, sugerindo quadro de anemia hemolítica imunomediada a qual pode estar associada secundariamente à hemoparasitose (OLIVEIRA, 2020). O fenômeno autoimune, nestes casos, pode ocorrer por autoanticorpos contra a membrana eritrocitária parasitada, havendo a destruição das mesmas pelo sistema monocítico fagocitário, ou lise celular por ação do sistema complemento e reação de hipersensibilidade do tipo II, em que os anticorpos, principalmente IgG e IgM, são produzidos contra os hemácias do hospedeiros, promovendo uma diminuição do volume globular (GOLYNSKI, 2008). Tais enfermidades apresentam curso clínico rápido, associado a intensa hemólise e prognóstico reservado, evidenciando a importância de um diagnóstico laboratorial rápido e preciso.
