Shock presenting in emergency – Dengue or sepsis?

- Abstract
A significant number of patients present to emergency room with shock. In tropical countries, a sizeable number of these patients may be categorized into having either septic shock or dengue shock syndrome. It may be difficult to differentiate the two at presentation because of presence of fever in both. Consequently, many of the patients with dengue shock may be managed as per septic shock treatment algorithm. However, there are significant differences in the resuscitation protocols concerning type of fluids, transfusion of blood products, vasopressors, inotropes and corticosteroids. Emergency room physicians need to be sensitized regarding more aggressive blood transfusion protocols in dengue shock syndrome based on “three-dimensional” assessment of hematocrit levels. Erythrocyte sedimentation rate is an important investigational modality that can help in differentiation and should find a place in Emergency room evaluation of shock patients along with hematocrit assessments.
