- Abstract
Introduction: Ectopic eruption of the permanent first molar is a common local eruption disorder occurring during mixed dentition. It is described as an abnormal mesioangular path of eruption resulting in early resorption of the primary second molar. The prevalence ranges between 2% and 6%, with a higher incidence in the maxilla. It has multifactorial etiology and is diagnosed usually during the routine clinical and radiographical examination. Early intervention is important to prevent the mesial tipping of the upper first permanent molar and early loss of the primary second molar, thus preserving the arch circumference. Various treatment modalities have been mentioned in the literature for the management of ectopic eruption. Case Report: The present case report describes the management of an ectopically erupting maxillary first molar in a 10-year-old child with a modified Halterman appliance. The Halterman appliance was modified by incorporating cross-arch anchorage with a distal extension engaging chain elastic from the occlusal button on a permanent molar. An activation of the appliance was performed at 2 weeks interval, and successful distalization of the upper first molars was observed 6 weeks later. Clinical Significance: Clinical management of ectopic eruption is a demanding task for the pediatric dentist and is of great importance for the development of a healthy permanent dentition and stable occlusion. Modified Halterman appliance is an effective appliance in achieving distalization of an ectopically erupting permanent molar in the mixed dentition period.
