FOLIC ACID CAN INCREASE TETRAHYDROBIOPTERIN LEVEL IN PLASMA OF HYPERTENSIVE PATIENTS WITH HYPERHOMOCYSTEINEMIA BUT CANNOT RELIEVE INFLAMMATION.

- Abstract
Experimental and epidemiological evidences have suggested that homocysteine (Hey) may be a causal risk factor fo r atherosclerosis. Folic acid (FA) can reduce Hey and improve endothelial function in many short-term studies, but the effects of long-term FA treatmetit on different people may be distinct. A randomized, double-blinded, placebo-controlled intervention trial was performed to test the preventive effects of long-term FA supplementation (10 mg) on atherosclerosis in hypertensive patients with hyperhomocysteinemia. Hey, biopterins and carotid intima-medial thickness (CIMT) in 100 participants were detected at least one year later. After one year, the FA-treated group bad a significantly lower mean plasma homocysteine concentration than that of the control group (14.63 pmol/L versus 28.11 pmol/L, P<0.001). the level of tetrahydrobiopterin (BH4), which obviously increased in the FA-treated group, decreased in the control group (4.31 nrnol/L versus 3.88 nmol/L, P<0.001). Lhe neopterin concentration increased in both groups, but the post-treatment CIMT (0.96 ± 0.09 mm FA versus 0.97 + 0.09 mm placebo, P=0.80) did not differ significantly. However, the improved endothelial function associated with homocysteine-lowering treatment was significant in short-term studies but not in longer trials. Although long-term supplementation with FA ivas associated with increased BH4 and decreased homocysteine, it did not reduce neopterin or CIMT. In other words, FA cannot reduce inflammation reaction or delay atherosclerosis development.
