- Abstract
A síndrome de cushing (SC), hiperadrenocorticismo ou disfunção da pars intermédia da pituitária (PPID), é uma afecção caracterizada pela perda de inibição dopaminérgica da pars intermedia da hipófise, que acomete geralmente animais idosos. Foi atendida uma égua da raça Crioula, 10 anos de idade, pesando 400 kg, com relato de emagrecimento progressivo havia cerca de 15 dias, incoordenação ao caminhar, polidipsia e pelagem com aparência hirsuta. Na inspeção, visualizou-se perda do tônus da cauda e de massa muscular na região lombar e glútea, pelos longos de aparência hirsuta, opaca e quebradiça. Além disso, apresentava pequenos abscessos na região tóracolombar, sendo que não apresentava outras alterações no exame físico. Suspeitando-se de SC, foram realizados exames laboratoriais de sangue e urina, nos quais foram encontrados linfopenia, leucopenia, hiperproteinemia, hiperglobulinemia leve, aumento das enzimas hepáticas GGT e FA, além de hipercolesterolemia e hiperglicemia. A urinálise evidenciou glicosúria, presença de hemácias, leucócitos e bactérias. Considerando os achados, iniciouse tratamento com enrofloxacina, por oito dias, associado à penicilina benzatina por seis dias. A fim de confirmar a suspeita de SC, realizou-se teste de supressão com dexametasona em dose baixa (40 mg/kg), o qual revelou níveis de cortisol pré-aplicação de 7,90 µg/dL, 9,00 µg/dL 15 horas após a aplicação e 12,50 µg/dL após 19 horas da aplicação. Nesse momento, realizouse avaliação radiológica dos cascos, a qual não revelou sinais de laminite. Com o diagnóstico de SC, iniciou-se o tratamento com ciproeptadina (0,25 mg/kg), VO, SID. Após 10 dias, a paciente recebeu alta hospitalar, sendo recomendada a continuação do tratamento na propriedade. Segundo o médico veterinário da propriedade, o animal vinha apresentando melhora, porém 10 dias após a alta, apresentou complicações respiratórias e após três dias foi a óbito. Os principais achados da necropsia incluíam pneumonia granulomatosa difusa acentuada, além de adenoma de pars intermedia da hipófise, esse último corroborando a suspeita clínica de SC. Os sinais clínicos apresentados nesse caso se enquadram nos sinais mais comuns de SC descritos na literatura, como hipertricose, atrofia muscular, letargia, laminite, poliúria, polidipsia, abdômen pendular e imunossupressão com infecção secundária. Existe uma variedade de testes para SC, sendo um deles o teste de supressão com dexametasona, que é de fácil execução. Valores de cortisol maiores que 1 µg/dL após as 19 horas da administração são sugestivos de SC. No presente caso, o nível de cortisol após 19 horas foi 4,6 µg/dL maior que o basal, confirmando o diagnóstico de SC. Tendo em vista que as endocrinopatias são pouco relatadas em equinos, ressalta-se a importância deste relato como descrição de sinais clínicos, diagnóstico e evolução clínica da SC em equinos de meia idade.
