COVID-19 Olgusunda Kötü Prognostik Komplikasyon: Spontan Pnömomediastinum ve Deri Altı Amfizemi.

- Abstract
Giriş: COVID-19 hastalığı ve komplikasyonlarının saptanması ve uygun şekilde tedavi edilebilmesi için nadir ama mortal olabilen klinik seyirleri vurgulamak önemlidir. COVID-19 takibinde barotravma olmaksızın pnömotoraks veya pnömomediasten hatta daha nadiren her ikisinin birlikte oluşması, mekanik ventilasyona bağlı komplikasyon olarak görülebilir. Takibinde spontan pnömomediastinum ve deri altı amfizemi gelişen mortal seyreden bir olgumuzu sunmayı amaçladık. Olgu: Kırk beş yaşında erkek hasta 3 gündür olan karın ağrısı ve ishal yakınması ile acil servise başvurdu. Ülseratif kolit tanısı ile azatiopürin tedavisi almakta idi. Günde 10 kez volumü az, non-mukoid ve hemorajik olmayan diyare tariflemekte ve ateş öyküsü yoktu. Genel durum orta, bilinç açık, ateş 36,5 °C, kan basıncı 110/70 mmHg, oda havasında parmak ucu oksijen satürasyonu (SpO2 ) %94, nabız 87/dk, solunum sayısı 18/ dk idi. Barsak sesleri artmış, diğer sistem muayeneleri olağan idi. İlk başvuruda laboratuvar tetkiklerinde tam kan sayımında beyaz küre 16,3 (109 /l), lenfosit 0,6 (109 /l), hemoglobin 12 (g/dl), platelet 405 (109 /l) idi. Biyokimyasal tetkiklerde kreatinin 0,9 (mg/dl), AST 174 (u/l), ALT 96 (u/l), albümin 28,5 (g/l), LDH 679 (u/l), ferritin 338 (ug/l), CRP 88 (mg/l), D-dimer 0,65 (ug/ml) idi. Gaita bakıda eritrosit, lökosit parazit kist ve yumurtası görülmedi. Akciğer grafisinde bilateral akciğer parankiminde yamasal dağılım gösteren alveoler infiltrasyon ile uyumlu opasiteler olduğu görüldü. Hastadan alınan COVID-19 PCR testi pozitif saptandı. Takibinde 3. gün hipoksisi olması üzerine SpO2 >%93 olacak şekilde nazal O2 desteği ve metilprednizolon başlandı. Yatışının 4. gününde rezervuarlı maske ile 10 lt O2 desteğinde SpO2 %92, solunum sayısı 30/dk saptandı. Servikalde ve torakal apekste şişlik, palpasyonda krepitasyon mevcuttu. Solunum sesleri azalmıştı. Kontrol yüksek çözünürlüklü toraks BT tetkikinde; boyundan başlayarak derin yumuşak dokuda, paraspinal kas planları komşuluğunda, üst-orta ve posterior mediasten boyunca yaygın hava imajları izlendi (Şekil 3a, b, c, siyah oklar). Hastada şiddetli COVID-19 pnömonisi ve ciddi akut respiratuvar distress sendromu PaO2 /FiO2 oranı <100 saptanması üzerine göğüs cerrahisi olan 3. basamak hastaneye sevk edildi. Yakın takip ve tedavisine rağmen yatışının 3. haftasında kaybedildi. Sonuç: Pnömomediastinum, COVID-19 pnömonisinin olası komplikasyonu olup, altta yatan akciğer hastalığı olan veya olmayan hastalarda ortaya çıkabilmektedir. Kötü prognostik bulgu ve akut dekompansasyon göstergesi olacak pnömomediastinum, göğüs ağrısı ve nefes darlığı gelişen hastalarda akılda tutulmalıdır.
