Estudio transversal de función cognoscitiva en pacientes con diabetes mellítus; tipo 2 con correlación metabólica y por imagen de resonancia magnética.

- Abstract
Este deterioro cognoscitivo leve en pacientes con diabetes se ha denominado como encefalopatía diabética o disfunción cognitiva diabética e incluso se ha diferenciado en encefalopatía diabética primaria causada por hiperglucemia y alteración en la acción de la insulina, además tiene relación con el tiempo de evolución de la hiperglucemia y se asocia con pérdida neuronal apoptótica y deterioro cognoscitivo; en contraparte la encefalopatía diabética secundaria parece ser resultado de enfermedad microvascular subyacente o bien como consecuencia de hipoglucemia. Objetivo: determinar si existe asociación y/o correlacionar entre las concentraciones de glucosa sérica, insulinemia, resistencia a la insulina, presión arterial sistólica, índice de masa corporal y enfermedad de pequeño vaso en encéfalo por imagen de resonancia magnética con la función cognoscitiva medida. Material y métodos: se evaluaron 48 pacientes consecutivos con diabetes mellitus y cuyo diagnóstico de ingreso no incluyera enfermedad vascular cerebral, trastorno neuropsiquiátrico, ni alguna otra enfermedad neurológica asociada a deterioro cognoscitivo. Se les realizó determinación de glucosa sérica, insulina basal, colesterol total, triglicéridos; así mismo se realizó IRM y una batería de pruebas neuropsicológicas con MMSE, COGNISTAT, NPI y IQCODE. Resultados: la edad media fue de 56.2 +/- 11.5 años de edad, relación hombre:mujer de 1:1.2, escolaridad media de 7 +/-3 años, tiempo medio de diabetes mellitus de 6.8 +/- 5.8 años; 53% de los sujetos son también hipertensos y 35% se conocían ya con dislipidemia. El IMC medio es de 28 +/- 4.4. La presión sistólica media es de 125.5 +/- 13.6 mmHg, glucosa media es de 164 +/- 69 mg/dl y HbAlc media es de 8.7 +/-2.3%, colesterol medio de 201 +/- 43 mg/dl y triglicéridos de 231 +/- 148, HOMA-IR medio es de 1.2 +/- 0.8. No se encontró correlación estadísticamente significativa entre los factores metabólicos y puntaje en las pruebas neuropsicológicas; sin embargo, si existe correlación entre la presencia de enfermedad de pequeño vaso y un decremento en las siguientes áreas del COGNISTAT: atención (r= -0.472, p=.001), comprensión (r= -0.352, p=0.014), repetición (r= - 0.338, p=0.19) y razonamiento analógico (r= - 0.298, p= .04). También hubo correlación entre la presencia de enfermedad de pequeño vaso y la puntuación total del COGNISTAT (r= -0.338, p=0.019) y MMSE (r= -0.417, p=0.003) no así con el IQCODE. Mediante prueba de U de Mann Whitney se encontró significancia estadistica entre la presencia de enfermedad de pequeño vaso y los dominios de atención (p=0.002), comprensión (p=0.02), construcción (p=0.033) y razonamiento analógico (p =0.041) únicamente. Discusión: en este estudio no se logró demostrar asociación entre los factores metabólicos y la función cognoscitiva mediante los instrumentos elegidos; sin embargo, si se demostró relación con la enfermedad de pequeño vaso. Conclusión: la enfermedad de pequeño vaso presentó en el 30% de los pacientes diabéticos de forma subclínica en esta muestra y se relaciona con decremento en la función cognoscitiva.
