Efecto de la craniectomía, descompresiva sobre el estado funcional y sobrevida de pacientes con deterioro rostrocaudal.

- Abstract
El síndrome de deterioro rostrocaudal representa la vía final común de diversas patologías del sistema nervioso central (SNC) que provocan hipertensión intracraneal. Hasta la fecha se considera a la cirugía de descompresión como una alternativa para mejorar el pronóstico de sobrevida y funcional de pacientes con hipertensión intracraneal. Objetivo: determinar el probable beneficio de la cirugía de descompresión externa en pacientes con deteñoro rostrocaudal en fase mesencefálica tratados entre 1998 y 2005 en el Instituto Nacional de Neurología y Neurocirugía Manuel Velasco Suárez. Material y métodos: se incluyeron a 136 pacientes con diagnóstico de deterioro rostrocaudal secundaria a EVC isquémico (n=28) y hemorrágico (HSA aneurismática [n=28], MAV's (n=6), y hemorragia parenquimatosa [n = 29]), neoplasias intra y extraxiales (n= 26), procesos infecciosos (n =7), y TCE (n = 11), que requirieron de craniectomía descompresiva. Se revisaron los expedientas de los pacientes y se determino el estado funcional de los pacientes en el preoperatorio y en el periodo posoperatorio mediante las escalas de Karnofsky, Barthel y Rankin modificada. Se realizo un seguimiento de entre 3 y 10 años de los pacientes incluidos. Se determino la sobrevida de los pacientes y RR de mortalidad de acuerdo al método de Kaplan-Meier y Log Rank (se excluyeron del an&lisis los pacientes con diagnóstico de gliomas grado III y IV de la OMS). Mediante curvas ROC se establecieron puntos de corte para evaluar el beneficio de la craniectomía descompresiva de acuerdo a los puntajes de las escalas funcionales. Se establecieron como puntos de corte para un desenlace de mal pronóstico puntajes menores a 60 en la escala de Karnofsky, Barthel < 50 y Rankin > 2 (AUC >95%, Sens > 98%, Esp > 95%, p < 0.001). Se determino la reducción absoluta del riesgo de mortalidad (ARR [1 C95%]), y el número de pacientes necesario a tratar para determinar el beneficio del procedimiento quirúrgico (NNTB IC95%[NNTB]). Resultados: en el periodo preoperatorio se encontro un mayor puntaje en las escalas de Karnofsky y Barthel en comparación con las evaluaciones del periodo posoperatorio. (35.14 ± 1.29 vs 77.64 ± 3.10 y: 20.99 ± 2.37 vs 62.97 ± 3.80, respectivamente; p < 0.001). No se observaron diferencias estadísticamente significativas en la mortalidad y en el estado funcional de los pacientes en función de la lateralidad del hemisferio afectado. Se observo una disminución en el riesgo de mortalidad de los pacientes dependiente del estado funcional a su ingreso al quirófano. El número de pacientes necesario a tratar para beneficiar a los pacientes fue de 2 IC95% [1.8-4]. Se observo una disminución significativa de la mortalidad de los pacientes dependiente del estado funcional a su ingreso. Conclusiones: la craniectomía descompresiva es un procedimiento quirúrgico seguro y eficaz para el tratamiento de la hipertensión intracraneal.
