- Abstract
The eye is considered to be a vital organ and its involvement in autoimmune rheumatic diseases may be sight-threatening. The ocular manifestations can be markers of active systemic disease as well. The article underlines from a clinical perspective the types of ocular involvement in spondylarthritides, rheumatoid arthritis, juvenile idiopathic arthritis, dermatomyositis, main primary vasculitides and antiphospholipid syndrome. The onset of ocular disease can be sudden or insidious. Eye involvement may be symptomatic or quiescent. The therapy has to be individualized. Keratitis and episcleritis usually benefit from anti-inflammatory therapy, while serious ocular manifestations of SLE (such as scleritis and lupus retinopathy) generally require systemic immunosuppression. Early recognition by the rheumatologist, prompt referral to the ophthalmologist and coordinated treatment strategies are important to reducing the ocular morbidity associated with these diseases.
