115: Comparison between Short and Long Term Results of Open Prostatectomy and TURP in Different Prostatic Weight Groups.

- Abstract
Introduction: Prostatectomy for patients with BPH is one of the most frequent operations. The relative impact of the two surgical techniques (open prostatectomy versus TURP) on quality of life seems to be different. Surgical outcomes and impact of the two aforementioned surgical techniques on short and long term quality of life were compared and appropriate prostatic weight for each technique was determined. Methods: 246 men with BPH and severe urinary symptoms were included in the study. They were classified according to the sonographically determined prostatic weight into 5 groups: less than 40 g, 40 t0 49 g, 50 to 59 g, 60 to 69 g and 70 g or more. In each group (except for group with prostatic weight less than 40 g which all underwent TURP) patients were randomized to undergo open prostatectomy or TURP. We determined preoperative characteristics, perioperative complications, early and late postoperative outcomes, 6 and 80 months after surgery respectively. Results: Preoperative characteristics and perioperative complications revealed no significant difference between two surgical methods in each prostatic weight group. Early postoperative outcomes were similar in both methods. IPSS, 80 months after surgery, was significantly higher in patients with larger prostatic weights who underwent TURP, especially when greater than 60 g. Conclusions: Improvement of urinary symptoms occurs with both TURP and open prostatectomy techniques. Among patients with prostatic weight larger than 60g open prostatectomy seems to be more effective in long term.
