- Abstract
A letter published in the journal Techniques in Coloproctology discusses the use of cutting seton (CS) in the management of anal fistulas. The letter highlights that CS is associated with higher postoperative pain and longer healing time compared to other treatment modalities, while having similar recurrence and incontinence rates. The authors argue that there is a lack of evidence to support the assumption that CS preserves the sphincter muscle and prevents incontinence. They also note that CS is commonly used in South Asian countries, where sphincter loss is frequently observed. The authors suggest that further research, including randomized controlled trials, is needed to evaluate the impact of CS in high fistulas and to dispel the belief that CS is safe in all anal fistulas.
