Laparoscopic vs. Open Gastrectomy for Advanced Gastric Cancer in a Western Population: a Propensity-Score Matched Analysis of Surgical and Oncological Results.

Laparoscopic gastrectomy (LG) is a well-established procedure in Asia. Data on LG in Western countries are scarce, where patients tend to present with more advanced disease and proximally located tumors. We aimed to compare the outcomes of LG vs. open gastrectomy (OG) for the treatment of AGC in a Western population. Single-institution propensity score-matched retrospective study of consecutive patients operated on AGC, either open or laparoscopic, from January 2010 to December 2019 at a European tertiary hospital. Endpoints were postoperative complications and oncological outcomes. A per-protocol analysis was performed. After propensity-score matching, 153 patients were included, 96 for OG and 57 for LG. There were no differences between the OG and LG groups in postoperative complications (38.5% vs. 35.1%), the number of retrieved lymph nodes (24 [IQR: 15–38] vs. 26 [18.3–34.8]), or recurrence (39.6% vs. 38.6%). No significant differences were found in DFS and OS with a follow-up of 43.5 months (IQR: 24.35–63.2). LG is safe and feasible for the treatment of advanced gastric cancer in a Western population, without differences regarding postoperative complications and similar short-term oncological results.
