- Abstract
Patients with schizophrenia criticize the low availability of psychotherapy as a common form of stigmatization. In this article, we investigate whether patients with schizophrenia are undersupplied with psychotherapy and whether they are underrepresented in treatment settings compared to other patient groups. Furthermore, we elucidate in what way structural and individual stigmatization contribute to this underrepresentation. First, we review the evidence for availability of inpatient and outpatient psychotherapy. Next, we elucidate structural causes and evidence of individual stigmatization by healthcare professionals. Finally, we present studies investigating the association of health care professionals' stigmatizing attitudes and treatment-related decisions. The studies show that the majority of patients with schizophrenia are not provided with psychotherapy. While the data was insufficient to evaluate an underrepresentation in inpatient settings, data on outpatient treatment indicates an underrepresentation of patients with schizophrenia. Possible structural causes include the perpetuation of erroneous assumptions about schizophrenia during therapists' education and training and by decision makers, and organizational factors within the healthcare system. Further individual-based potential causes are stigmatizing attitudes among healthcare professionals, especially prognostic pessimism. Stigmatization in the health care system exists on an individual and a structural level. We discuss potential interventions to change this deficiency.
