Isolated premature atrial contractions in pediatric patients: Nine years single-center experience.

- Abstract
Objective: In this study, we aimed to review our experience about diagnosis, treatment, and follow-up of isolated premature atrial contractions (PACs) in pediatric patients. Material and Methods: A retrospective review was conducted through our electronic database used in our pediatric arrhythmia center. Between November 2011 and November 2020, 101 patients >1 month and <18 years of age, having no significant heart defect/cardiac tumors, and with isolated PACs were enrolled in the study. Patients with inadequate follow-up/missing data were excluded from the study. Results: The mean age of the total 101 patients (60 males; 59.40%) was 10.52±4.51 years, and the mean weight was 38.42±18.82 kg. Mean PAC number on the first Holter by admission was 7676±9032 and mean PAC frequency was 7.06±6.69%. Sixty-six patients were asymptomatic (65.34%). PAC with aberrations was seen in 30 patients (29.70%), and non-conducted PAC was seen in seven patients (6.93%). In 24 patients, trademill exercise test was performed, and in 14 of them, no PAC was diagnosed at all or PACs fully disappeared during exercise. Medical antiarrhythmic therapy was initiated in only nine patients with frequent PACs, who were symptomatic. During a mean 28.84 months of follow-up (5–88 months), PACs disappeared totally in 22 patients (21.78%), PAC frequency has decreased in 50 patients (49.50%), PAC frequency did not changed in 15 patients (14.85%), and PAC frequency increased in only 14 patients (13.86%). Forty-four patients (43.56%) were excluded from follow-up, by whom PACs were fully or near fully disappeared at the end. Conclusion: In this study, we found that isolated PACs in pediatric patients are mostly asymptomatic, and detected incidentally. Most of them fully or near totally disappeared in follow-up and do not require any treatment. In a few patients with frequent PACs, being symptomatic, antiarrhythmic treatment can be used without any serious side-effects.
