- Abstract
Introdução: A sialolitíase consiste na obstrução dos ductos das glândulas salivares, devido à calcificação do conteúdo salivar, podendo causar sintomatologia dolorosa, edema, processos infecciosos e necrose das células parenquimatosas glandulares. Clinicamente é raro que essas lesões ultrapassem a medida de 10mm de diâmetro. É uma patologia comum que acomete aproximadamente 1% da população, predominantemente adultos de meia idade. Apenas 5% a 20% dos casos ocorrem na glândula parótida. O manejo adequado, através de litotripsia ou remoção cirúrgica conservadora, são imprescindíveis para evitar complicações. O diagnóstico definitivo pode ser obtido através de exames imaginológicos associados ao exame clínico e análise histopatológica. Objetivos: Relatar um caso clínico de sialolitíase localizada incomumente no ducto da glândula parótida. Conduta Clínica: Paciente do sexo feminino apresentou dor e inchaço na face, com evolução de 60 dias. Relatou sentir gosto amargo e salgado. Sob ectoscopia, havia edema e eritema no lado direito da face. A oroscopia revelou saída de exsudato purulento pelo ducto de Stensen. A radiografia evidenciou um corpo radiopaco. A hipótese diagnóstica foi de sialolitíase intraductal parotídea. Realizouse excisão cirúrgica e "ordenha", irrigação com soro fisiológico e instalação, no ducto glandular, de dreno de acrílico confeccionado por impressão 3D, a fim de evitar obstrução, por 7 dias. A análise microscópica mostrou fragmentos de tecido desmineralizado, acelular, irregular, ora basofílico, ora eosinofílico confirmando o diagnóstico de sialolitíase. Resultados: A paciente retornou 7 dias depois para remoção do dreno, com completa cicatrização e sem sinais de inflamação. Permanece sob proservação. Conclusão: A sialolitíase deve ser considerada como hipótese diagnóstica quando o paciente apresentar edema facial na região da parótida. A excisão cirúrgica e colocação de dreno para evitar obstrução podem ser consideradas condutas importantes, associadas a bom prognóstico.
