- Abstract
Zusammenfassung Etwa 3% aller duktalen Pankreaskarzinome beruhen auf einer vererbten genetischen Prädisposition. Hereditäre Formen des Pankreaskarzinoms treten dabei in 3 klinischen Szenarien auf: 1. Tumorprädispositionssyndrome, die mit einem erhöhten Risiko für ein Pankreaskarzinom einhergehen, wie z. B. das Peutz-Jeghers-Syndrom und das familiäre atypische multiple Mole-Melanom-Syndrom. 2. Die hereditäre Pankreatitis sowie die zystische Fibrose prädisponieren zu der Entwicklung eines Pankreaskarzinoms aufgrund einer frühen chronischen Entzündung der Drüse. Das 3. und häufigste Szenario ist das familiäre Pankreaskarzinomsyndrom (FPC). Entsprechend einer Konsensuskonferenz sollten Hochrisikopersonen für die Entwicklung eines Pankreaskarzinoms in Früherkennungsprogramme einbezogen werden. Da der Benefit eines Früherkennungsprogramms derzeit noch nicht eindeutig belegt ist, sollten derartige Programme nur im Rahmen von prospektiv kontrollierten Studien an Expertenzentren durchgeführt werden. Eine prophylaktische Pankreatektomie ist basierend auf der derzeitigen Datenlage nicht indiziert, da der zugrunde liegende Hauptgendefekt des FPC bis dato unbekannt und die Penetranz des Pankreaskarzinoms bei den anderen prädisponierenden Tumorsyndromen niedrig bzw. nicht abschließend geklärt ist. Sollten jedoch ein Pankreaskarzinom oder hochgradige Vorläuferläsionen diagnostiziert werden, erscheint eine prophylaktische Erweiterung des Resektionsausmaßes zur totalen Pankreatektomie sinnvoll, da Patienten mit hereditärem PC häufig multifokale Läsionen entwickeln.
