- Abstract
OBJECTIVE: Pancreatic injuries following abdominal trauma are seen rarely compared to other solid organ injuires.Isolated pancreatic injury were reported less than %5 of all blunt abdominal trauma. Imaging fi ndings of pancreatic injuries may be diffi cult to diagnose.Mortality and morbitiy rates increases because of late diagnosis. In this report aims to present computer tomography (CT) fi ndings of a case with isolated pancreatic injury following blunt abdominal trauma. MATERIAL AND METHOD: 27 years old man who presented to the emergency department because of blunt abdominal trauma. The patient has pain at epigastrium and lost consciousness. Hemoglobin and hematocrit levels were low,amylase value was high.Abdominal ultrasound showed a fl uid collection 5 cm in diameter which has not vascular fl ow with doppler ultrasound. Intravenous contrast enhanced CT of abdomen was performed because to explain source of this fl uid collection. RESULTS: CT showed a hematoma which measured 5*15 cm in diameter near pancreatic corpus and there was transection of parenchyma in this area.(fi gure 1-2,red arrow)Splenic artery didn't injure but splenic vein was compressed by hematoma. According to The American Association for the Surgery of Trauma (AAST) injury scoring scale, this pancreatic injury was diagnosed type IV laceration. The patient underwent surgery and diagnosis was confi rmed. CONCLUSION: Pancreatic injuries are uncommon but mortality and morbidity rates are high. However, ultrasound is fi rst step imaging protocol for trauma patients, pancreatic and other retroperitoneal injuries may not be diagnosed truly.CT is reliable and uniqe for early diagnosis which is very important for trauma patients
