- Abstract
The article discusses the challenges of treating chronic limb-threatening ischemia (CLTI) and the need for improved drug delivery in below-the-knee (BTK) revascularizations. The SAVAL trial, which compared a paclitaxel-eluting stent to balloon angioplasty for infrapopliteal occlusive disease, did not show a benefit for the stent. The failure of drug-eluting stents (DES) in this trial raises questions about the factors that contributed to the lack of success, such as the narrow range of treatable vessel diameters and the potential for early thrombosis. Despite these setbacks, the author believes that progress will eventually be made in drug delivery for BTK vascular disease.
