- Abstract
Introducción: La obesidad es un problema de desequilibrio de nutrientes que se traduce en un mayor almacenamiento de alimentos en forma de grasa, que los requeridos para satisfacer las necesidades energéticas y metabólicas del individuo. Cuantitativamente se define como IMC > 30 kg/m² en el adulto y por encima del 97 percentil en los niños. Objetivos: Determinar el comportamiento evolutivo de los niños grandes quemados obesos. Material y método: Se realizó un estudio descriptivo, prospectivo, longitudinal y correlacional en la unidad de quemados del Hospital Pediátrico Universitario "Paquita González Cueto" de la ciudad de Cienfuegos, Cuba, en el periodo comprendido entre el 1 de enero del 2010 al 31 de diciembre del 2011. El universo estuvo constituido por 78 pacientes ingresados en la unidad en el periodo de tiempo antes mencionado, tomándose como muestra los pacientes que presentaban evaluación de la relación peso/talla por encima del 97 percentil y con los pronósticos de grave, muy grave, crítico y critico extremo, quedando constituida por 23 pacientes para un 29,5%. Resultados: Predomino el sexo masculino y el grupo de edad de 6 a 10 entre los quemados obesos, igual predominio del sexo masculino y del grupo de 6 a 10 entre los quemados no obesos, lo que nos muestra compatibilidad entre los grupos a comparar, de los 23 pacientes obesos del grupo de estudio el 69,6% presento complicaciones de tipo infeccioso y el 100% de los críticos y críticos extremos tuvieron además complicaciones cardiovasculares y renales, el 78,3% de los pacientes obesos presentaron alguna complicación durante su evolución aun con pronósticos de grave en porcentajes elevados, de los 33 pacientes del grupo de los no obesos solo se complicó el 36,4% con bajos porcentajes en los grupos de menor pronóstico, solo el 42,4% de los enfermos de este grupo presentaron alguna complicación a lo largo de su evolución, Todos los pacientes de este estudio (obesos y no obesos) egresaron vivos. Conclusiones: Los pacientes obesos portadores de quemaduras extensas se complicaron más que los no obesos en este estudio.
