NUTRICIÓN PARENTERAL DOMICILIARIA: REGISTRO NACIONAL 2006.

- Abstract
Objetivos: Presentar los datos sobre nutrición parenteral domiciliaria (NPD) en nuestro país durante el año 2006. Material y métodos: Estudio realizado mediante la base de datos del grupo NADYA (www.nadya-senpe.com) del nuevo registro de pacientes con NPD. Resultados: Se han comunicado 75 pacientes (56% mujeres) de 16 hospitales, con 78 episodios de NPD. El número total de días de tratamiento fue de 19.434. La edad media en los lt; 14 años fue de 7 ± 8,49 DE y de 53,41 ± 15,19 en los > 14 años. De ellos, 37 pacientes conservaban ingesta oral de alimentos y 9 recibieron nutrición enteral complementaria. El 18% de los pacientes recibieron NPD por un tiempo < a 3 meses, 17% 3-12 meses, 26% 1-2 años y 40% > 2 años. En los < 14 años los diagnósticos principales fueron: intestino corto traumático 33%, malformaciones congénitas intestinales 25%, alteraciones de la motilidad 17%, y colitis ulcerosa, fibrosis quística y SIDA 8%, respectivamente. Entre los > 14 años fueron: cáncer 37% (23% paliativo, 14% activo radical), isquemia mesentérica 17%, enteritis rádica 11%, alteraciones de la motilidad intestinal 9%, enfermedad de Crohn 2%, otras 24%. El 44% utilizaron un catéter central tunelizado, 28% un reservorio y otras 28%. Las complicaciones más frecuentes fueron las infecciosas (0,8 episodios/1.000 días de catéter) y las metabólicas. A lo largo del año finalizaron 16 episodios de NPD (44% por pasar a dieta oral/enteral, 31% por fallecimiento). El 49% de los pacientes presentaban una actividad normal, estando limitada en el 35% de los casos. Entre los adultos, el 59% eran autónomos y el 41% requerían ayuda total o parcial. El hospital de referencia suministró la nutrición parenteral en el 88% de los casos y el material fungible en el 83%. Sólo el 19% de los pacientes fueron considerados por sus médicos candidatos a trasplante intestinal. Conclusiones: Con los datos de nuestro registro la prevalencia de NPD en España es de 1,7 pacientes/106 habitantes, inferior a la media de los países europeos. Aunque la patología benigna continúa siendo el diagnóstico principal en nuestra serie, se observa un incremento del número de pacientes con cáncer (principalmente en tratamiento paliativo). En la mayoría de los casos la NPD se utilizó por un período superior a 12 meses. Hemos observado un descenso en la tasa de infecciones respecto al año anterior.
