- Abstract
Introduction. The purpose of this study was to evaluate the utility of running preoperatory a high-resolution CT-scan in cases of middle-ear cholesteatoma. Methods. We conducted a prospective study on a 18-month period, between January 2015 and July 2016, on a number of 35 patients with the diagnosis of cholesteatoma hospitalized in the ENT Department of the “Iuliu Hațieganu” UMPh, Cluj- Napoca, who were surgically treated using CWU or CWD maneuver, followed by tympanoplasty with TORP or PORP titanium prostheses. The aspect of ossicular lesions and the extension of cholesteatoma were evaluated with both intraoperative and postoperative CT. Results. Regarding the localization, in cases with attic and mastoid involvement, we had a good correlation with CT image (100%), in all other cases the extension of cholesteatoma being more significant on the CT image, probably because of the granulation tissue. As for the correlation of the bone destructions (CT aspect vs. intraoperative aspect), this was good, especially regarding the hammer bone and incus. The utility of preoperative CT was indubitably demonstrated in revealing complications such as lateral semicircular canal dehiscence or dural dehiscence (100%), and less in highlighting the facial nerve impairment (20%). Conclusions. The high-resolution CT-scan proved to be usefull in diagnosing and establishing the therapeutic conduct in patients with cholesteatoma, providing information on the extension of the lesions, on the possible ossicular lesions and on the presence of possible complications. The CT-scan is usefull in assessing the extention of the cholesteatoma in difficult areas regarding surgery, such as sinus tympani or anterior attic.
