- Abstract
This article highlights the progress that Medicaid has made in rebalancing the nation's long-term care (LTC) system for older adults, from nursing homes to home- and community-based services (HCBS). Participation and expenditure trends (1999-2008) for three Medicaid HCBS programs that serve elders (home health, waivers, and personal care services [PCS]) show an increase over the period. Wide variations in HCBS participants and expenditures occur across states. Data from a national survey of states show an increase in waiting lists for older adults between 2002 and 2010, indicating growing unmet needs and continued demand for such services compared to nursing homes. Although progress has been made in expanding HCBS to 60 percent of total LTC participants, only 30 percent of LTC spending was for HCBS in 2008.
