- Abstract
Chronic venous insufficiency (CVI) poses a health challenge, impacting vascular efficiency and the condition of the skin on the lower extremities. CVI symptoms, especially in stage C6 with active ulcers, require targeted intervention but often undergo prolonged conservative therapy. Aim of the study is to illustrate how the lack of causal treatment can delay the therapy for venous ulcers in CVI stage C6. The choice of prolonged conservative therapy hinders the swift implementation of necessary procedural treatment for effective healing of venous ulcers. The cases of two patients, aged 67 and 65, with active skin ulcers in stage C6 CVI, are described. These patients had previously undergone prolonged conservative treatment without significant improvement. Improvement was achieved only by implementing procedural treatment after referring the patients to a specialized vascular diseases clinic. In both cases, endovascular laser ablation (EVLT) combined with foam sclerotherapy was applied, leading to the complete closure of diseased veins and healing of ulcers. The results underscore the challenges associated with the prolonged conservative treatment of active venous ulcers, advocating for the economic and therapeutic benefits of targeted interventions such as EVLT and foam sclerotherapy in accelerating the healing of venous ulcers and improving the quality of life for patients. There is a need to increase awareness, interdisciplinary collaboration, and the availability of diagnostic tools to optimize the care of patients with CVI. Chronic venous insufficiency, especially in conjunction with active ulcers (C6), represents a significant societal and individual burden, and effective interventions can substantially improve treatment outcomes.
