The article discusses the development of a cause and effect diagram. Enumerated are steps on creating a cause and effect diagram which include defining the problem, identifying causes of the problem and drafting the diagram. Tackled is the empirical research on root cause analysis (RCA) as well as the elimination of root cause hypotheses.
