- Abstract
Objetivo: conocer si la longitud telomérica (LT) y la variante de riesgo BDNF_p.Val66Met presentan relación con el funcionamiento cognitivo de pacientes con esquizofrenia. Antecedentes: los pacientes con esquizofrenia presentan síntomas positivos, negativos, y alteraciones cognitivas que afectan su funcionalidad y calidad de vida. Se ha sugerido un patrón de degeneración cognitivo asociado con un envejecimiento acelerado. En este sentido, se ha demostrado una LT (marcador de envejecimiento biológico) más corta en pacientes con esquizofrenia vs. controles. El alelo Met de la variante rs6265 del gen del factor neurotrófico derivado del cerebro, BDNF, se asocia con una actividad reducida y también se ha relacionado con funcionamiento neuropsicológico más deficiente. Escasos estudios han explorado la asociación de LT con el desempeño cognitivo y variantes genéticas en pacientes con esquizofrenia. Métodos: se incluyeron 41 pacientes con esquizofrenia y 45 controles sanos pareados. La LT leucocitaria se cuantificó mediante qPCR y método comparativo, y el genotipo de la variante rs6265 de BDNF_p.Val66Met por discriminación alélica. La cognición se evaluó con la batería Matrics Consensus Cognitive Battery. Los resultados clínicos-moleculares-psicológicos se analizaron estadísticamente en R. Núm. de registro del protocolo: INNN_38/19 Resultados: la LT de pacientes fue menor que la de controles (p=0.04); mostrando una tendencia con la respuesta al tratamiento y el tipo de antipsicótico (primera vs. segunda generación, p=0.04). El desempeño neuropsicológico fue significativamente menor en pacientes vs. controles (p<0.001). Observamos correlaciones moderadas significativas entre la LT y el desempeño en atención/vigilancia (r=-0.343, p=0.03) y con memoria de trabajo (r=-0.304, p=0.05) en los pacientes. Al clasificarlos por antipsicótico, la correlación entre LT y atención/ vigilancia permaneció solo para pacientes tratados con olanzapina. En relación a la variante de riesgo de BDNF_p.Val66Met no encontramos ninguna correlación con LT o desempeño cognitivo en general o por dominio. Conclusiones: la LT puede ser un biomarcador de deterioro cognitivo (atención y memoria de trabajo) en pacientes con esquizofrenia.
