Manejo de la depresión en pacientes con epilepsia: estudio controlado de TCC contra tratamiento farmacológico.

- Abstract
En México hay alrededor de 2 millones de pacientes con epilepsia y más del 42% sufre de depresión. La depresión en epilepsia tiene características propias, su efecto sobre la calidad de vida (CV) puede ser más devastador que la misma epilepsia, y es un factor que incide negativamente sobre el curso y control de las cdsis. Hasta ahora, el tratamiento de la depresión en la epilepsia se ha enfocado al manejo farmacológico, con riesgos por interacciones farmacodinámicas y farmacocináticas con los antiepilépticos. La terapia cognitivo conductual (TCC) es un procedimiento psicoterapéutico activo, directivo, estructurado y de tiempo limitado cuya efectividad en el tratamiento de diversos trastornos psiquiátricos, incluyendo la depresión, ha sido probado. Hasta ahora no existen estudios para determinar su eficacia en pacientes con epilepsia. Objetivos: determinar el efecto de la TCC sobre los síntomas de depresión y calidad de vida, en pacientes con epilepsia, en comparación con el tratamiento farmacológico antidepresivo habitual. Material y métodos: se realizó un ensayo clínico controlado con pacientes de la consulta externa de diversos servicios del INNN, con diagnóstico de epilepsia del lóbulo temporal y síntomas de depresión. A partir de un muestreo no probabilistico se formaron dos grupos. El primero recibió TCC de grupo (n=20) en 16 sesiones semanales; el grupo control recibió tratamiento antidepresivo farmacológico (n=22). El diagnóstico psiquiátrico se hizo através de la entrevista estructurada MINI. La severidad de la depresión y CV se evaluaron con el IDB y la escala de calidad de vida en epilepsia QOLIE 31. Los instrumentos de evaluación se aplicaron antes de iniciar el tratamiento, a las 8 semanas y al final del tratamiento. Resultados: el análisis del modelo lineal para muestras repetidas, mostró que el efecto sobre los síntomas de depresión tanto para el tratamiento farmacológico como para el tratamiento psicoterapéutico, es estadísticamente significativo (F=15.9; p<0.000), ambas maniobras mejoraron los síntomas de depresión, observándose el máximo efecto en las pñmeras 8 semanas. Al comparar ambos grupos, existe una diferencia estadísticamente significativa entre la mejoría que experimentaron los pacientes que recibieron el fármaco antidepresivo (FAD) y los que recibieron TCC (p <.000, F 181.03). No se encontraron diferencias estadísticamente significativas en CV, no obstante el grupo que recibió TCC tuvo una media más favorable en la QOLIE31. Conclusionas: la TCC mostró ser una opción efectiva para el tratamiento de los síntomas depresivos en pacientes con epilepsia. Los pacientes que recibieron el tratamiento farmacológico tuvieron un mal apego al mismo y más efectos adversos. Se requieren de otras investigaciones que confirmen estos resultados y de ensayos clínicos controlados contra otras intervenciones grupales para descartar que la mejoría observada no se deba únicamente al efecto del apoyo grupal.
