Presentación clínica y radiológica de la neurocisticercosis pediátrica y sus diferencias con los casos adultos.

- Abstract
Antecedentes: la neuroctsticercosis (NCC) se caracteriza por su gran heterogeneidad clínica dependiendo del número de quistes que se instalan en el sistema nervioso central, su localización, su tamaño, su estado de degeneración, así como de la intensidad de la reacción inflamatoria asociada. Factores del hospedero podrían influenciar cada uno de estos parámetros. En particular, existen algunas observaciones que señalan la posible relevancia de la edad. Objetivos: evaluar la relevancia de factores etarios en la presentación clínica y radiológica de la neurocisticercosis. Material y métodos: se Ilevó a cabo un estudio retrospectivo en el cual se reviso los expedientes clínicos y radiológicos de los pacientes con NCC confirmada por imagen, atendidos en el Instituto Nacional de Pediatría y en el Hospital Infantil de México durante los últimos 10 años. Se recabaron las características clínicas e imagenológicas de los pacientes y se analizó si el género, la edad, así como la intensidad de exposición al parásito participaban en la heterogeneidad de la presentación de la NCC. Los resultados fueron comparados con los datos reportados por Fleury et al., (2O04) en un estudio equivalente realizado en el Instituto Nacional de Neurología y Neurocirugía. Resultados: en la población pediátrica, la sintomatología más frecuente fue la epilepsia (78.4%) mientras que la presentación radiológica más usual fue un parásito único (57.3%) en estadio coloidal (42%). La localización preferencial fue en el parénquima cerebral (96%). Los niños provenientes de zonas rurales (alta exposición) presentaban más lesiones múltiples que los niños provenientes de zonas urbanas (60% vs 30.8%, P=0.04). Existían diferencias significativas en la presentación de la NCC entre los niños y los adultos: en la población adulta, se encontró más frecuentemente múltiples parásitos (77.1% vs 43% en adultos y niños respectivamente, P<0.0001), vesiculares (27.6% vs 4.2%, P<0.0001) localizados en el espacio subaracnoideo de la base (50.5% vs 0% P<0.0001). Conclusiones: estos resultados confirman la relevancia de factores etarios en la expresión clínico-radiológica de la NC. El estudio de los mecanismos inmunológicos, endocrinológicos y patológicos involucrados, permitirá un mejor entendimiento de la patología y orientará hacia un mejor manejo de esta enfermedad cuya morbilidad es elevada en países endémicos.
